New Patient Forms
If you are a new patient, please arrive 20 minutes early to complete the necessary paperwork. To save time, you may download the forms below and fill them out in advance.
Office Hours & Appointments
Our offices are open Monday through Friday from 8:00 a.m. to 5:00 p.m. Patients are seen by appointment. Same-day and next-day appointments are available for urgently ill patients.
Please bring your insurance card to each visit and notify us of any changes in your address, phone number, and/or insurance information.
To assist your physician, new patients and patients with a new prescription are asked to bring all medications (prescription and non-prescription) to each visit.

Payment for Services
All co-payments, coinsurance, and deductibles are due at the time of service. If you receive services that are not covered by your insurance, you will be responsible for them. We accept cash, check, MasterCard, and Visa.
Proof of identification and insurance should be provided before seeing a provider. If you are unable to provide us with proof of insurance at your visit, we will consider you a self-pay account and expect payment in full at the time of service.
If your insurance coverage changes, please notify us as soon as possible so we can make appropriate changes to our information to maximize your benefits.
We will submit your insurance claims and assist you in any way that we can reasonably help to get your claims paid. Your insurance company may need you to supply them certain information directly. It is your responsibility to comply with their request.
Prescriptions
If you have any questions concerning your condition, treatment, or medications, please call your physician's office. If you need a medication refill, call your pharmacy. There may be a 48-hour turn-around time on your prescription refill
Referrals
Referral authorizations may take up to five business days to process, so please contact your PCP as soon as the need for a referral arises.
Cancellations
We ask that that you arrive for your appointment at 20 minutes prior to your scheduled time or reschedule appointments you are unable to keep 24 hours prior to appointment time. Missing multiple appointments without notice could result in a charge or you being dismissed from our practice. If you are late, you may be asked to reschedule. This will allow us to serve other patients.
